Help on identification, old amp.

Couldn't tell you the type. Looks to be 4 similar tubes plus one smaller. Looked up some Elkin pics; I think this is what it is. The switches on my unit are only labeled on/off. Knobs are unmarked. Some one added labels for ant/radio.

Just picked this up. In decent shape aside from some obvious tinkering. No tag/ID on this unit.
