On the switching supply front everyone knows they are electronically noisy and they always have been. It is a matter of of if the noise is where you talk and listen that matters.
You can reduce the noise some and move it around but you will never get it all out.

Most amateur radio's have a pretty wide audio bandwidth a D104 would effectively degrade that. Is that on purpose? Plus the Icom has +8V on the cord you could either power the unit or you could replace the mic element with and electret and power it from the radio.
You would be much better off all of you guys with a decent signal bipolar transistor amp or duel transistor amp. Even if they are HG's! That is not worth the money or the time.

That chasis can not dissipate that much heat! I would never push a radio that hard.
I think that is a recipe for failure with non-rf mosfets! I know the parts are cheap but shipping and labor add up in a hurry.

You said old! I do not deal with old tuning transformers that had mica cap built in but as I understand it they can get mold growing on them??? I might be misunder standing this. I have seen green poly caps grow something white that splits them open.
Mica all the way! The use of ceramic caps unless we are talking a doorknob is normally about cost. You would see CB amps loaded with ceramics because they are cheap! That is even more true today.
