Since I did a clean install of Vista a few weeks ago, the only issue I have is when the computer woke up after going to sleep or standby, it had audio stuttering until I rebooted it. Solution is not to let it go to sleep, or know that it will need a reboot if it does.
I've had success running CCleaner to cure some stuttering/slowing/lagging types of issues on a couple of machines.
Sometimes the only thing that helps is a clean install of Windows.:headbang
